Log in to your WordPress admin panel. 2. Go to Plugins > Add New. 3. Search for “SimpleForm”. 4. Click Install Now, then Activate. 5. After activation, go to SimpleForm > Forms in the admin menu to build your first form. 6. Copy the shortcode from the form list and paste it into any page or post. ## FAQ ### Can I send WordPress form submissions to WhatsApp? Yes. SimpleForm includes native WhatsApp integration. When a visitor submits a form, they can be redirected to WhatsApp with the form data pre-filled in the message body. This works on mobile and desktop, and no paid addon or third-party account is required. ### Can I receive form notifications in Telegram? Yes. You connect a Telegram bot to SimpleForm by entering your bot token and the chat ID of the channel, group, or user you want to receive notifications. After that, every submission triggers a Telegram message with the form field data. This is included in the free version. ### Does SimpleForm sync submissions to Google Sheets? Yes. Connect your Google account via OAuth in the Integrations settings, pick a spreadsheet, and map form fields to columns. New submissions create new rows automatically. The OAuth token management was overhauled in version 3.9.0 to resolve previous connection issues. ### Can I get form submission alerts in Slack? Yes. Enter your Slack webhook or configure the Slack integration under Integrations, select a channel, and SimpleForm will post a notification for each new submission. You can set different channels for different forms. ### How do I create a quiz with scoring? Go to SimpleForm in the WordPress admin, open the Quizzes section, and click Create Quiz. Add your questions, mark the correct answers, set point values, and configure the pass/fail threshold. You can add images to questions and answers, set a time limit, and enable randomization. Display the quiz on any page with `[simple_quiz id="X"]`. ### Can I set a time limit on a quiz? Yes. In the quiz settings, enter a time limit in minutes. When the timer runs out, the quiz submits automatically and shows the results. ### How do polls work? Create a poll under the Polls section, add answer options (single or multiple choice), configure voting rules, and embed it with `[simple_poll id="X"]`. You can restrict voting to logged-in users, set a closing date, and control whether results are visible to voters before or after they vote. ### Can I create multi-step forms? Yes. Add a page break field between sections in the form builder to split the form into steps. A progress indicator appears automatically. You can choose from several progress bar styles and customize the button labels for each step. ### Does conditional logic work with multi-step forms? Yes. Conditional logic applies per-field and works across steps. You can show or hide a field on step 3 based on what a user selected on step 1. ### Can I replace the WordPress login page with a custom form? Yes. Build a custom login form in SimpleForm, then go to Settings > Auth Settings and enable the option to replace the default WordPress login form. You can also replace the registration form, the comment form, and the WooCommerce login and registration forms independently. ### Can I export form submissions? Yes. Open the Leads page and export all submissions as CSV, JSON, or PDF, or print them directly.
